POSITION SUMMARY: Performs concurrent and retro chart reviews for quality initiatives, such as core measures, regulatory compliance and other outcome databases. Collects, aggregates, analyzes, and reviews data for improvement opportunities. Collaborates with and educates staff to achieve compliance and assists with hospital wide performance improvement processes. ESSENTIAL JOB RESPONSIBILITIES: 1. Perform concurrent review, data collection and analysis through clinical chart reviews. Based on indicators, investigate cases referred, as required or requested. Perform inter-rater reliability (IRR) abstraction for core measures. Conduct reviews in a timely manner to meet review and reporting requirements. Reviews include but not limited to: CMS/JC Core Measures reviews; e.g. AMI, HF, PN, STROKE, VTE, SCIP, etc. Other quality initiatives, e.g. CHOIS complication & mortality review, NCDR - Get with the Guidelines, Mission Lifeline, STS, etc. Analyzes and reports data from clinical outcomes systems such as CHOIS, Core Measures, etc. Joint Commission/Regulatory compliance, e.g. audits, tracers, etc. Perform targeted/focused performance improvement review as requested. Maintain strict confidentiality of all data and information. 2. Promote performance improvement by providing information and follow up. When possible, perform concurrent contact with physicians to document orders, notes, etc. as directed by physician to achieve compliance. Collaborate with providers and staff to achieve compliance. Collaborate with clinical educators and other staff to provide education and serve as resource regarding requirements. Ensure appropriate and timely communication to achieve compliance. Report summaries of review results, feedback on potential quality issues and trends, peer review to Quality Director. Perform back-up abstraction for performance improvement activities; e.g. peer review, quality reviews, etc. Assist with coordination of the quality and performance improvement processes as directed by the Quality Director. Assist with regulatory and accreditation compliance. Maintain strict confidentiality of all data and information. 3. Complete other duties assigned. Maintain databases and review sheets as required. Meet deadlines and timeframes for reviews and reporting. Develop or review policies and procedures as required. Complies with all hospital policies and procedures, safety requirements, and emergency preparedness requirements.
